What to bring to a sliding-scale mental-health appointment in Houston: a bilingual, printable checklist covering common income questions, ID uncertainty, medication lists and what to confirm directly with the provider.

Bring proof of income if you have it, a photo ID if available, your medication list, insurance or Medicaid information, and the provider’s appointment details. If any document is missing, call before your Houston appointment and ask what alternatives they accept, because sliding-scale and identification rules vary by provider.

Confirm the fee before you travel

“Sliding scale” usually means the amount you pay depends on income or household circumstances. It does not automatically mean the visit is free, and each provider may calculate the fee differently.

Ask these questions by phone:

  • What will my first appointment cost?
  • How do you calculate the sliding-scale fee?
  • Is there a separate charge for an evaluation, medication management, paperwork, or lab work?
  • When is payment due?
  • Do you accept cash, cards, or payment plans?
  • Will I need to prove my income again later?

  • ¿Hay otro cargo por la evaluación, el manejo de medicamentos, documentos o análisis?
  • ¿Cuándo tengo que pagar?

Write down the staff member’s answer and the date you called. If the quoted price is more than you can pay, say so directly and ask whether another fee, assistance program, or referral is available. Gather any income documents you already have

A provider may ask about your income to decide your fee. Bring documents you already have, but do not assume every office accepts the same proof.

Possible documents include:

  • Recent pay stubs
  • A benefits letter
  • An unemployment statement
  • A recent tax return
  • A bank statement
  • A letter from an employer
  • A written statement explaining that you currently have no income

The clinic may also ask how many people live in your household or depend on that income. Ask how the provider defines “household,” especially if you live with relatives or roommates but do not share money.

Do not send sensitive financial documents by text or ordinary email unless the provider confirms that method and you are comfortable using it. Ask whether you can bring paper copies or show the documents during check-in.

Call if you do not have photo ID

Bring a photo ID if you have one. If you do not, call the provider instead of canceling the appointment or assuming you will be turned away.

Use this script:

“Hi, I have a sliding-scale mental-health appointment on. I do not have a current photo ID. Can I still be seen? What other document can I bring?”

En español:

“Hola, tengo una cita de salud mental con tarifa según mis ingresos el [día]. No tengo una identificación vigente con foto. ¿Todavía me pueden atender? ¿Qué otro documento puedo llevar?”

Ask whether the office accepts an expired ID, school ID, consular ID, benefits card, a document showing your name, or another form of identification. These are questions, not promises. Only the provider can tell you what it accepts.

Make one complete medication list

Write down everything you currently take, even if another doctor prescribed it. Include:

  • Prescription medications
  • Over-the-counter medicines
  • Vitamins and supplements
  • Medication allergies or serious past reactions
  • The dose, if you know it
  • How often you take each item
  • The name and phone number of your pharmacy

You can bring the bottles, take clear photos of the labels, or write the information on paper. Do not stop or change a medication because of a checklist or website. Discuss medication questions with a licensed clinician.

If you need a refill soon, write down how many doses remain. Ask whether the person you are seeing can prescribe medication or whether you need a separate appointment with a medical prescriber.

Make one final confirmation call

Call one or two business days before the appointment. Confirm the time, address, fee, accepted income documents, ID requirements, language support, cancellation policy, and whether the clinician provides therapy, medication management, or both.

If nobody answers, leave only the information needed for a callback. Avoid describing private mental-health concerns in voicemail unless you choose to do so.

Put the provider’s number, address, quoted fee, and required documents on one sheet of paper. Keep that sheet with your medication list so the essentials are ready when you leave.
