The morning a Houston restaurant worker realizes a clinic may charge more than today’s tips: a bilingual 30-second checklist for asking about sliding-scale fees, uninsured visits, and required documents by phone.

If a Houston clinic visit may cost more than you can spare today, call before you leave and ask three things: the price for an uninsured visit, whether a sliding scale can lower it, and which documents you must bring. The script below takes about 30 seconds in English or Spanish.

Have three details ready

Before calling, write down:

  • Your ZIP code.
  • The reason for the visit in a few words, such as “sore throat” or “refill question.”
  • Whether you have Medicaid, another insurance plan, or no insurance.

You do not need to give a long medical history to ask about cost. Do not share immigration information. For the price question, “I do not have insurance” is enough.

If you have severe chest pain, trouble breathing, signs of a stroke, or another immediate danger, call 911. A pricing call should never delay emergency help.

Say:

“Hi, I need an appointment for [short reason]. I do not have insurance. What is the price for an uninsured visit? Do you offer a sliding-scale fee? What documents should I bring, and do I need to apply before the appointment?”

Then ask one final question:

“Is there any other charge I should expect today?”

That last question matters. The quoted visit price may not include lab work, medicine, imaging, or a procedure. The clinic may be unable to predict every charge, but you can ask what the basic visit includes.

If the answer moves too quickly, say:

“Could you repeat the amount and the document list slowly, please?”

Write down the staff member’s answer, the date, and the time of the call. Clinic policies and hours can change.

Use this 30-second checklist in Spanish

Di:

“Hola, necesito una cita por [motivo breve]. No tengo seguro médico. ¿Cuánto cuesta una consulta para una persona sin seguro? ¿Ofrecen una tarifa según los ingresos? ¿Qué documentos debo llevar y tengo que solicitar el descuento antes de la cita?”

Después pregunta:

“¿Hay algún otro cargo que podría pagar hoy?”

Si necesitas que hablen más despacio, di:

“¿Puede repetir el precio y la lista de documentos más despacio, por favor?”

También puedes comenzar con:

“¿Hay alguien que hable español?”

Spanish availability can vary by location, shift, or day. Ask whether a Spanish-speaking staff member or interpreter will be available during your appointment, rather than assuming the person answering the phone will also be there later.

Ask what “sliding scale” means at that clinic

A sliding-scale fee changes according to household income or another clinic policy. The phrase alone does not tell you what you will pay.

Ask for the amount that applies to you:

“What might I pay based on my household income and household size?”

In Spanish:

“¿Cuánto podría pagar según los ingresos y el número de personas en mi hogar?”

The clinic may need documents before giving an exact amount. Some offices may offer an estimate by phone. Others may require a financial-assistance application at check-in. Ask whether the discount applies to the first visit or only after approval.

Do not describe a sliding scale as free care. A reduced charge can still be difficult to cover when rent, food, transportation, and today’s tips all come from the same pocket.

Confirm the required documents

Document rules differ by clinic. Ask the office to name each item instead of guessing.

Possible requests include proof of income, proof of address, identification, a Medicaid denial letter, or information about household size. These are examples, not a universal checklist.

If you are paid in cash, have irregular tips, or do not have a standard pay stub, ask:

“What can I bring if I do not have that document?”

In Spanish:

“¿Qué puedo llevar si no tengo ese documento?”

Also ask whether copies or phone photos are accepted. That detail can prevent a second trip.

If a staff member asks for something you do not understand, request the exact name of the form and ask whether the clinic can help you complete it. Asking about financial documents does not require you to discuss immigration status.

Compare the full cost before choosing

Call at least two clinics when your condition allows you to wait safely. Record four answers for each one:

  • The uninsured visit price or estimated range.
  • The sliding-scale process.
  • The documents required.
  • The earliest available appointment.

A lower quoted price may come with a later appointment, a longer bus ride, or separate lab charges. A closer clinic may save transportation costs or missed work. Compare the whole trip, not one number.

CaminoCare can help you identify nearby safety-net clinics by ZIP code and check which listings report uninsured access, sliding-scale fees, Medicaid acceptance, or Spanish availability. Make the call before your shift

Open your phone’s notes and paste the English or Spanish script. Add your ZIP code and short reason for visiting. Call the first clinic, record the four answers, then call a second if time and symptoms allow.

Before hanging up, repeat the key details back:

“So the visit is estimated at ____, I should bring ____, and my appointment is _____. Is that correct?”

That short confirmation can protect today’s tips, prevent a wasted trip, and help you arrive with what the clinic needs.
